State officials have suspended the license of Cornerstone Living Center of Winston-Salem, saying conditions there endanger the health, safety and welfare of its 76 residents.

The Winston-Salem Journal reports notices were sent by certified mail and hand-delivered Tuesday to the center, located on Reynolds Park Road. An official with the N.C. Department of Health and Human Services said Cornerstone's license would be suspended by 5 p.m. on June 2 or when the last placement of a resident to another center has been arranged.

State action came after a resident survey and an investigation of a complaint made May 18.

There have been several recent incidents where residents with mental or cognitive impairments walked away from the center. At least two criminal incidents have been reported, including the robbery of drugs and an allegation that a resident was a victim of rape.

Cornerstone had no comment on the state's actions.

Under state regulations, Cornerstone has 20 days to appeal the suspension.
